This form must be completed and returned before tickets will be issued to ensure that all information such as, frequent flyer numbers, credit card numbers, travel dates and name spellings are recorded correctly. You must provide us with your name, as it appears on the documentation you plan to present at the airport. It is strongly recommended that you submit your request as soon as you know your dates of travel. Press the "Submit" button at the bottom of this form upon completion.
Once we receive your request a reservation is made, an itinerary and pricing sent to you via email for approval. Please approve or provide a change request immediately to avoid the loss of space. All tickets will be issued in accordance with applicable fare class rules. Once issued, tickets are subject to applicable cancellation and change penalties. No charges will be made to your credit card without written authorization approving the itinerary and pricing that has been submitted to you.
